A: The hypertension and diabetes assessment, mental health screening, healthcare facility matching, care navigation and referral support, and appointment coordination.

**Q: What does care navigation mean?**

A: A real person explains your results, finds a suitable facility for your budget and location, books the appointment, and follows up with reminders.

**Q: What is not included?**

A: Assessment and package fees do not include tests, scans or consultations at the facility, which you pay there by cash, card, mobile money or insurance.

**Q: How do I get started?**

A: Request a callback on this page and a care navigator calls you within 2 hours, Monday to Saturday, 8am to 8pm.
